Immigration Medical Exam I-693

I-693 immigration medical exam in Houston, TX with a USCIS-authorized physician. Vaccine review, lab work and the form in a sealed envelope.

Anyone applying for permanent residence from inside the country needs Form I-693, the immigration medical exam report. At Clínica Hispana Cruz 4 it's performed by a USCIS-designated physician, with the whole process explained in Spanish or English.

How the process works here

At the first visit the doctor reviews your history, does the physical exam, compares your vaccine records with what the CDC requires for your age and draws samples for the tuberculosis, syphilis and gonorrhea tests that apply. Once every result is back and your vaccines are complete, you sign the form in front of the doctor and we hand it to you in a sealed envelope to send to USCIS.

What to bring

  • Passport or another photo ID
  • Every vaccine record you have, from here and from your home country
  • A medication list and reports on any chronic conditions
  • Glasses, if you wear them
  • If you've had a positive TB test before, that report or the chest X-ray

About vaccines

Every record you bring is a dose you won't have to repeat. The flu shot in season and Tdap are given at the clinic. If you're missing another one, we tell you which and where to get it, and we record it once you bring proof.

Before you book

USCIS limits how much time can pass between the doctor's signature and filing your application, and that rule has changed in recent years. Confirm the current one with your attorney or on the official I-693 page before coming in, so the exam doesn't fall outside the window.

One important detail

Don't open the envelope. USCIS only accepts it sealed exactly as we hand it to you.

Frequently Asked Questions

Usually two: one for the exam and samples, and one to sign and pick up the envelope once results are in. A missing vaccine or a positive test can add a step.

Yes, as long as they show the date and the vaccine name. Bring them even if they're in Spanish or on an old card; the doctor reviews them and records them on the form.

A chest X-ray is ordered. If it's clear, the process continues. If there's any doubt, the doctor refers you to the health department to finish the evaluation.
