Every August, Houston schools and sports teams ask for the same thing: a recent physical and a form signed by a doctor. At Clínica Hispana Cruz 4 we take care of it without an appointment, explaining each step in Spanish to both the student and the parents.

What the doctor checks

Weight, height and blood pressure
Vision, and hearing when the form asks for it
Heart and lungs, listening for murmurs or irregular rhythms
The spine, to catch curvatures such as scoliosis
Joints, strength and flexibility before sports
History: fainting or chest pain during exercise, asthma, allergies and relatives with early heart problems

Forms we complete

The pre-participation evaluation form Texas schools require for UIL sports, school enrollment, daycare and summer camp forms, and soccer, baseball or basketball league forms. Bring the printed form with the parent section already filled in; the visit goes faster.

If the school asks for more

The TB test some programs require is a skin test: it's placed at this visit and read 48 to 72 hours later, so plan on one return trip. The Tdap vaccine, which Texas requires for seventh-grade entry, we give right here. For other school vaccines we review the shot record and tell you which are missing and where to get them.

Seasonal tip

July and August are the busiest months. If your child already knows their sport or school, coming in June skips the line and leaves time in case the doctor wants an extra check before signing.
